The worst one I can think of is Silence of the Lambs. The depiction of Buffalo Bill being mentally ill and talking like a retarded person could be offensive. His motive seemed strange when all he had to do was wear women's clothing and put make-up on his face.

Psychos operate in all walks of life and Buffalo Bill had many other issues.
I haven’t read the book, but he presented as male, his sick proclivities were private and as explained, his pathology was a thousand times worse than a generic trannie.
