How exactly did the "cell phone hit" get set up?

Anyone else wondering how the "cell phone assassination" on the judge worked? Did Clyde rig the phone super-tiny explosives or something? And how would he have gotten access to her phone in the first place?

More so how did he do it just at that right time???
Did he stuff explosives in it then program it so when his number came in that's when it would explode.

Clyde was already in solitary at the time the Judge was killed. He was probably in the courthouse and swapped her phone before the meeting took place. He probably had the Judge's chambers wired and knew everyone's routines. You don't need a lot of explosive to kill someone at close range, so some C4 or Semtex rigged to a small blasting cap connected to a duplicate of her phone, dial the number and poof.
