I am remembering a plotline, but can't recall the names of any characters involved, or what season it was in.
A woman who had lived on Wisteria Lane years before comes back, along with her teenaged daughter and elderly mother. However, there were hints that her departure from the neighborhood had been sudden and unexpected by the neighbors. The elderly mother keeps trying to tell the teenaged girl something, but the mother keeps drugging her and eventually smothers her in her bed in an attempt to prevent it. Another girl in the neighborhood who had been friends with the daughter when they had been little is confused when the daughter doesn't remember her. Eventually it comes out that the teenaged girl who this woman has raised as her daughter is not the daughter she had actually given birth to, but a child she'd adopted from a Russian orphanage after her birth daughter had been killed in a household accident (a cabinet had fallen over and crushed her when she had climbed on it trying to retrieve a toy). Was this actually a DH plot or am I confusing with some other show?? I've looked through the plot summaries and not seen anything that sounds like this -

As poster above mentioned, it was the Katherine character, but it was actually her elderly aunt lily, not her mother. Katherine doesn't smother Lily; she dies a natural death (she was sick) and manages to get the truth written down before she does.
Also, I think it was a Romanian orphanage
By the way, what a chilling reveal that was. As to WHY Katherine would ever do such a thing as she did, well, we'd have to ask the DH universe PTB
