    Kathy Batesno one could come close to topping this stellar performance of a psychotic Annie Wells (I think that's her name)But if Kathy Bates had not been available at the time who could have played her up to par for at least the part of a raving psychopath?
    I've got my sights set on Roseanne Barr. Of course back then she was a little younger looking that Bates, but she would have worked. She wouldn't have been as psychotic as Bates, but once again this is with Bates out of the spotlight in context only. I would pick Bates over anyone in consideration for the part.
    Rosie O'Donnell if she could lay off the lesbian thing could do it too. And Donald Trump could play Paul Sheldon!

      I think Angelica Huston would have been pretty good in the role if scheduling hadn't gotten in the way and forced her to pull out. She has a certain coldness and aloofness that would be great in the scenes when Annie totally disconnects from reality and goes into one of her depressions.
      I could also see Sissy Spacek in the role. She would have brought a much more delicate quality that would have made the insanity and intensity pretty frightening.
      However, Kathy Bates was brilliant in the part and a case of pitch perfect casting. I think physically she is so close to what was described in the book that it is very frightening.

        The only problem I have with Anjelica Houston as Annie (this will sound awful), but she's toopretty. She's a very glamourous woman, and Annie really isn't a glamourous woman (she even states that in the movie). Annie's supposed to be kind of matronly and unattractive. Don't get me wrong, I adore Kathy Bates, but she's not going to be playing Morticia Addams anytime soon.
        Although now that you mention it, Sissy Spacek really would've been a terrifying Annie.

          That is true, Anjelica is a very glamorous woman. However, she has such strong features, almost masculine features, that with the right hairstyle, clothing, and makeup, it would be easy to make her glamour almost freakish, thus giving her while not a plain look, a very odd, off-putting look.

                  I loved Kathy Bates in this. She played the part to Perfection like most probably no one else would but this is my list of alternatives:

                  1. Jamie-Lee Curtis
                  2. Susan Sarandon
                  3. Dolly Parton (She would be truly great in this type of horror flick)
                  4. Glenn Close (Maybe)
                  5. Kate Winslet

                        Sally Field is a thought, I think she could've nailed the personality. My only concern would be believing that she could be a dominating 'physical' presence as Kathy was 'cause she's such a tiny lady. She could've gained some weight and wore clothes with lots of padding or something, but still.
                        And obviously Meryl Streep, but she could play anybody.

                            Susan Sarandon sure has the potential to look psychotic with her large, expressive eyes but I think that, fundamentally, the actress would need to be slightly porculent. Just as Paul Sheldon's character needs to be of a slight build (more easily overpowered by his captor).
                            Oh and it's Annie Wilkes, not Wells (to the OP)
                            I think Tyne Daly sounds good - she's played such a character before. Glenn Close couldn't cut that sort of role - she's been there and done that in Fatal Attraction and is too slender IMHO. Kate Winslet, ironically, sounds a good choice as she can carry off all manner of characters (Think Heavenly Creatures, Titanic and Iris. She looked very different in all three movies and could easily gain a few pounds for a remake).

                                ellen burstyn, ABSOLUTELY. she wouldn't have been as convincing strength wise, as kathy bates, but she sure as hell can play crazy (requiem for a dream).
                                even though she would've been too young at the time, Winona Ryder comes to mind. The same goes for Shannen Doherty.
                                I think Oprah Winfrey could've pulled it off, she's scary enough, and would be believable in the fighting scenes since she's bigger.
                                Also Judi Dench and Marcia Gay-Hardin.
