Um, no offense, I'm not sure why you didn't get this: he is paying for "sex" w/ the Mexican prostitutes but it is just for essentially the connection w/ a woman, to feel her.
That first hooker he was with, she was faking it, that's what she was paid to do, as a hooker. Thus, you notice his intense and heartbreaking reaction: tears of anguish that he could not perform. And never would.

Yeah, I wondered that too. I figured either it can get hard, but he can't feel any sensation on it/around it so he can't move it or orgasm, or he wasn't actually inside her, and she was just grinding around on top of it, as it just lay there, flaccid. Personally, I was surprised she didn't do what most disabled guys in that situation do, which is have someone "milk" their prostate anally to induce orgasm, since they have no penile sensation. You are taking things too literally he wasn't castrated he still physically has everything but they don't work so they are useless to him thus him using the term "Castrated" and also saying he has a catheter in place of a penis because instead of the urine coming out of his penis it comes out of the catheter. Get it? -

I never got the impression he didn't have a penis, it just didn't work and he felt embarrassed about the fact when he was with the prostitute.
Although it's possible I guess that his penis was removed, cause it was never gonna get erect anyway, so it would be more simple to insert a catheter and remove it when needed? That seems a bit extreme but who knows. -

At the time of the film's release, Oliver Stone said that Kovic told him he "went down on" the prostitute.
Stone said it was a challenge to imply oral sex without having the MPAA wanna give him an 'X' rating.
But Stone specifically used the term, "cunnilingus", when describing the scene.
So, she was supposed to riding his face, I guess.
