Just got done watching this for the first time.
In addition to schizophrenia/split personalities, Leon's sexual repression seemed to be a major theme in this movie. When he was young, seeing the nurse have sex with Pin seemed to traumatize him in a way, as well as leave him with a distorted/confused impression about sexso for a while he then tried to suppress his natural sexual urges (The Need), but all this suppression only led to him having unhealthy, pent up, and incestuous feelings for his sister (the only girl he was ever close to). It was as though he just didn't want to acknowledge his sexuality outright, so he used poetry as a release for his feelings (writing a poem about a man named "Testes" who goes around having sex with hundreds of women in order to produce hundreds of offspring, and then who contemplates raping his own sister) He was a very creepy guy with a lot of mental issues.
