My opinion, the scene was literal, but this is an interesting take on it. I wouldn't rule it out.
The kid was always a nutter either way though. Some kinds are born, more than made. The specifics of his upbringing made him worse, but they didn't create his illness.

It helped a lot to make Leon believe that Pin was real and it contributed to his sexual repression because he was a kid when he saw that and it left a bad memory in his head about the whole idea of sex!
