When they pull the old lady out on her bed and leave her alive.
It's obviously a real old lady, and they have the flamethrowers burning down the buildings behind her..
Amazing to plan out a shot like that.

It's an unrealistic scene, not sure what the director was trying to say with it.

        Why was it unrealistic?
From what I heard he had done quite a bit of research for this movie.. Seems like a strange thing to just throw in there..
'Hey, let's get some old lady and get the Germans to take her out on her bed in front of the burning buildings just for fun'.

The men of the Waffen-SS were a pack of wild sadists and asking "why?" they did anything usually will not give anything valid as a response, especially concerning the units behind the front. Summarizing what the Waffen-SS officer says when he looks through the windows inside the barn and later when he speaks to the partisan commander it starts all over again with children, and women who can give birth. That is why they took the woman which tried to get out of the barn, with her child, to rape here and tossed her son back inside the barn. By raping her they (probably) ruined her chance of ever having another child while simultaneously leaving her son to be killed in the barn. Of course, they also did that because they wanted to have fun. But leaving the old lady while making a comment in the lines of you will still bare children, they were just being sarcastic.

Right. And I think it was also realistic that they were such "wild sadists" as you say, that they didn't seek to be absolutely thorough and efficient in following through on killing every last soul. The protagonist was shut in the barn to be burned alive; when he escaped, they toyed with him for a while, posed a photograph with a gun to his head, but then ultimately just didn't even bother to kill him.

Exactly! They had a "job" to do, but this was no ordinary task. They were still human but torn apart between indoctrinated hate and their human side. Drinking, raping, burning, destroying, they unleashed their worst as men, their beasts within, because they couldn't handle finishing this one job clear minded so they allowed their beast to take over, to transform everything into a game designed to feed their most sadistic urges. To look at this carnage through a prism of "fun and games" is perverted, but they managed it, because it really only replaced the other, true perversion they were tasked with, but made it something they could cope with. But this allowed military rules and discipline to be pushed aside and make way for one man's law.
