I imagine they can work out the speed the car was moving at by the depth and type of bone breaks/organ damage etc. Once the lungs are crushed it would mean a person would become unconscious very quickly (also what with the shock to the body, internal bleeding).
It's horrible to think of it happening, but I think the quicker the vehicle is moving, probably the better in terms of suffering. -
